With this attitude accidents would never be investigated. The puzzle pieces need to be put together, not to validate personal bias, but to learn lessons about how things happen and why so changes can be implemented that reduce or eliminate the factors that cause the accident. Hopefully the causes of this accident can be put together so that no other family has to go through something so awful.
